3. On receipt
of this information listed under No.1, ESA will issue an invoice,
a letter to the student, an "Appointment of Welfare Supervisor
and Consent" form and Homestay Application form, if required,
by email or, if preferred, by fax.

4.
The agent/client will then fax a copy of the cheque, bank draft, the direct
deposit slip or telegraphic transfer slip, along with the student’s details,
made out for the amount on the invoice to (613) 9775 2142

6.
Welfare Supervision needs to be accepted by one parent who must
sign the form and send it directly to ESA or to the agent, approving
the welfare supervision arrangement. This form must be
accompanied by a copy of the inside of the parent’s passport
and then be signed by Kenneth or Alison Taylor, taking responsibility
for welfare supervision and also providing a copy of the inside
of his/her passport.
7.
On receipt of proof of payment, ESA will notify the institution
that welfare supervision is in place and documentation will
be sent by the institution to the agent who can use it to apply
for the Student Visa.
8. A
copy of these documents should be kept in the student’s files at the
institution.
9. Students needing to change address once they
have commenced their studies, should advise a Director of ESA firstly, who will
in turn advise the institution re approval. The institution will then
issue a letter which the student can take to DIMIA.
